29 CFR 1926.501(b)(14):  Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening was less than 39 inches (1.0 m) above the walking/working surface, was not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system:      a)  Second and third floor window openings, west side of building, 517 Walnut Street, Hollidaysburg, PA 16648:  Observed on or about October 13, 2016, each employee walking/working near wall openings where the outside bottom edge of the wall opening was greater than 6 feet or more above lower levels was not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.  The inside bottom edge of window openings on the third floor of the west side of the building measured approximately 31 inches above the floor.  The inside bottom edge of window openings on the second floor of the west side of the building measured approximately 25 inches above the floor.  Employees working on the second and third floors on the west side of the building were exposed to a fall hazard of up to approximately 25 feet to the ground below.
