29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about October 17, 2016 and at times prior thereto, an employee was working on a work platform, eight feet in height without fall protection, exposed to the hazard of falling to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about October 17, 2016 and at times prior thereto an employee used a ladder that did not extend at least three feet above the upper landing surface to access a work platform. This condition exposed the employee to the hazard of falling to the ground below.
