29 CFR 1926.20(b)(2): The employer did not ensure that such programs provided for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employer: Instances including, but not limited to the following, the employer did not ensure that appropriate steps were taken to discover and correct safety and health hazards at the job site: a) Job site, at/near 1612 Graham Court, Pittsburgh, PA: On or about October 11, 2016, during construction activities, the employer did not ensure that subcontractor employees were working in a safe manner in that they were exposed to fall hazards, electrical hazards and were using ladders in an unsafe manner. b) Job site, at/near 1612 Graham Court, Pittsburgh, PA: On or about November 2, 2016, during construction activities, the employer did not ensure that subcontractor employees were working in a safe manner in that they were exposed to fall hazards, were exposed to electrical hazards and were using ladders in an unsafe manner.
