29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: a) On or about October 24, 2016, employees were exposed to head injuries when working below roofing operations with no head protection (hard hats) in place. Employees were working on the ground collecting roofing debris as it was being thrown from the roof above.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On or about October 24, 2016, employees where exposed to falls in excess of 16 feet while engaged in removing and replacing roofing materials from a roof with a pitch greater than 4/12 with no fall protection in place.
