OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the employees were exposed to struck-by and crushed-by hazards on Service Rig 138: a) On or about October 19, 2016, the employer did not ensure that two anchors had a visible tag securely attached to it indicating the capacity of the anchor and the date of the capacity verification (two anchors had tags that were out dated). b) On or about October 19, 2016, the employer did not ensure that the end terminations were in good working conditions. The guywires were not securely routed and properly positioned in the sheave.
