29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings: Basic Energy, Rig #1598: On or about October 20, 2016, the employer did not ensure that the rig's floor was equipped with standard railing when employees were engaged in well servicing operations. This condition exposed employees to fall hazards greater than 4-feet.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed: Basic Energy, Rig #1598: On or about October 20, 2016, the employer did not ensure that guarding was installed on four chains and sprockets installed on the ram and blowout preventer under the rig's floor. This condition exposed employees to caught between hazards.
