29 CFR 1910.176(a): Aisle(s) and passageway(s) were not kept clear and in good repair with no obstruction across or in aisles that could create a hazard: a. On or about October 24, 2016, inside the warehouse (vault), near the east bay door, the floor where mechanical equipment is used had a large hole with a loose plate covering a portion, exposing employees to a tip over hazard. b. a. On or about October 24, 2016, inside the warehouse (vault), near a blocked bay door on the north side, the floor where mechanical equipment is used had a large hole with a loose plate covering a portion, exposing employees to a tip over hazard. c. On or about October 24, 2016, inside the warehouse (vault), in the center of the of the east to west aisle, the floor where mechanical equipment is used had a large hole with ice cubes inside it and two cones near it, exposing employees to a tip over hazard. d. On or about October 24, 2016, outside the warehouse on the northeast side, the edge of the loading dock was in need of repair and was not maintained as designed, exposing employees to a tip over hazard.
