At the jobsite, a ladder or other safe means of egress was not provided and the employees were required to climb the north wall of the excavation, exposing employees to a fall hazard into the trench, on or about 10/25/2016.

29 CFR 1926.652(b)(1)(i): Excavations were not sloped at an angle not steeper than one and one-half horizontal to one vertical (34 degrees measured from the horizontal) and no other options listed in paragraph (b) of this section were used. a. On the north side of the excavation, sloping was measured at 57.9 degrees, exposing employees working in the excavation to an entrapment hazard, on or about 10/25/2016.
