29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:  a) Northwest corner of Basement Area A: A Schaefer pedestal fan used by the employees for ventilation did not have a continuous grounding path due to a two-prong replacement plug-end that was attached on the end of the three-wired cord, thus negating the grounding conductor.
