29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    a) Lower roof of Gym building. Employees engaged in roofing activities were not protected from falling approximately 14 feet to the ground below.

29 CFR 1926.1060(b): The employer did not provide retraining as necessary for each employee using ladders and stairways so that the employee maintained the understanding and knowledge acquired through prior training:  a) North side of the building at the lower roof edge: Employees showed that they had not retained the training they had been given on ladder safety as evidenced by having placed the ladder directly into the dirt, having no flagging at the upper access point, not securing it from movement, and placing a rope and anchor on it in such a way that it appeared to be tied off to prevent movement or displacement.   ABATEMENT CERTIFICATION IS REQUIRED
