29 CFR 1926.1060(b): The employer did not provide retraining as necessary for each employee using ladders and stairways so that the employee maintained the understanding and knowledge acquired through prior training: a) North side of the building at the lower roof edge: Employees showed that they had not retained the training they had been given on ladder safety as evidenced by having placed the ladder directly into the dirt, having no flagging at the upper access point, not securing it from movement, and placing a rope and anchor on it in such a way that it appeared to be tied off to prevent movement or displacement. ABATEMENT CERTIFICATION IS REQUIRED
