29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a) Southeast side of the roof of Building 144: An employee engaged in roofing activities on a 6/12 sloped roof was not protected from falling approximately 25 feet to the ground below.    Alternative Roofing Solutions, Inc.  was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(10)), which was contained in OSHA inspection number 1186518, citation number 1, item number 1 and was affirmed as a final order on (12-12-2016), with respect to a workplace located at 800 Shaw Drive, Shaw AFB, SC.

29 CFR 1926.502(f)(1)(iii): Points of access, material handling areas, storage areas, and/or hoisting areas were not connected to the work area by an access path formed by two warning lines:    a) Southeast side of the roof of Building 144: The point of access to the roof was not equipped with two warning lines running from the exit point of the extension ladder and to the work area.     Alternative Roofing Solutions, Inc.  was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1186518, citation number 2, item number 1 and was affirmed as a final order on (12-12-2016) with respect to a workplace located at 800 Shaw Drive, Shaw AFB, S.C.
