29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:  (Construction Reference 1926.103)    (a) An employee who voluntarily wore an N95 disposable filtering facepiece respirator while sweeping dust was not provided with the basic advisory information on respirators contained in Appendix D of the respirator standard.    Alternative Roofing Solutions, Inc. was previously cited for a violation of this Occupational Safety and Health Standard (29 CFR 1910.134(k)(6)) which was contained in OSHA inspection number 1175176, Citation Number 1, Item No. 1 and was affirmed as a final order on 10/24/16, with respect to a workplace located at FCI Bennettsville, 696 Muckerman Road, Bennettsville, SC 29512.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:    (a) The employer did not develop, fully implement, or maintain a written hazard communication program at the workplace where employees were exposed to gasoline and diesel fuel.    Alternative Roofing Solutions, Inc. was previously cited for a violation of this Occupational Safety and Health Standard (29 CFR 1910.1200(e)(1)) which was contained in OSHA inspection number 1175176, Citation Number 1, Item No. 2 and was affirmed as a final order on 10/24/16, with respect to a workplace located at FCI Bennettsville, 696 Muckerman Road, Bennettsville, SC 29512.

29 CFR 1910.1200(f)(6)(ii):  Except as provided in paragraphs (f)(7) and (f)(8) of this section, the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with product identifier and words, pictures, symbols, or combination thereof, which provided at least general information regarding the hazards of the chemicals, and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemicals:  (a) A 5 gallon metal safety can of gasoline located at the job trailer was not provided with an identity label.  (b) A 100 gallon tank of diesel fuel located in the back of a company truck was not provided with a hazard warning label.  ABATEMENT CERTIFICATION IS REQUIRED

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training as specified in 29 CFR 1910.1200(h)(2) and (h)(3) on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  (a)  An employee who was exposed to diesel fuel and gasoline had not been provided with effective information and training on hazardous chemicals as stipulated above.  (b) As of 02/06/18, at least one employee had not been trained on the new chemical container label elements and the new format and types of information on each section of safety data sheets (SDSs) with respect to revisions to the standard in accordance with the Globally Harmonized System of Classification and Labeling of Chemicals (GHS).  ABATEMENT CERTIFICATION IS REQUIRED
