29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace, including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form: Wing Headquarters Building: The employer did not conduct personal air monitoring to assess employee exposure to respirable dust containing silica generated during the partial demolition and reinforcing of CMU block walls inside the facility. Employees were wearing disposable filtering facepiece respirators (N95).

29 CFR 1910.1200(h)(3)(iv): Employee training did not include the details of the hazardous communication program developed by the employer, including an explanation of labels received on shipped containers and the work place labeling system used by their employer, and the safety data sheet, including the order of information and how the employees can obtain and use the appropriate hazard information: (a) As of 10/24/16, employees who were potentially exposed to silica and Portland cement had not been trained on the new chemical container label elements and the new format and types of information on each section of safety data sheets (SDSs) with respect to revisions to the standard in accordance with the Globally Harmonized System of Classification and Labeling of Chemicals (GHS). ABATEMENT CERTIFICATION IS REQUIRED
