29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) On or about October 25, 2016, Caesarstone USA, Inc. did not obtain the lift truck manufacturer's prior written approval before using a Vestil telescoping blue boom combined with an Aardwolf lifter on the Tusk sit down forklift (s/n 136317A). In accordance with 29CFR 1903.19(d), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: a) On or about October 25, 2016, Caesarstone USA, Inc. did not ensure the Tusk sit down forklift (136317A) was marked to identify the Vestil blue telescoping boom combined with an Aardwolf lifter and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered. In accordance with 29CFR 1903.19(d), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification.
