OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing and struck-by hazards while using an automotive lift:    (a)  On or about October 19, 2016, and at times prior, the employer did not ensure that employees were safe from crushing and struck-by hazards while using the 12,000 pound capacity Rotary Lift, Model SP012N510, in that the lifting pads on the swing arms did not contact the frame of Toyota Highlanders at the pinch welds that Toyota specified for lifting Highlanders, even with the manufactured riser attachments provided by the lift manufacture.  Toyota Highlanders were raised with unrated, unlisted and uncertified wooden blocks placed on the lifting pads of the swing arms.
