Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck-by inadequately secured loads.    On or about October 24, 2016, in the machine shop, where employees were exposed to struck-by hazards when hoisting material using a jib crane with a two-ton hoist without a safety latch on the hoisting hook.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.     On or about October 24, 2016, in the machine shop and fabrication shop, where employees were exposed to struck-by, caught-in and electrical hazards when performing service or maintenance on CNC lathe, Horizontal lathe, Vertical lathe and mill machines without implementation of an energy control program.

29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.    On or about October 24, 2016, and at times prior thereto, employees throughout the facility were required to operate forklifts without successfully completing formal and practical operator training.

On or about October 24, 2016:       a. In the machine shop where employees were exposed to struck-by and caught-by hazards when operating a Hypro brand vertical turret lathe without a guard to protect the operator from the rotating chuck.      b. In the machine shop where employees were exposed to struck-by and caught-by hazards when operating a Romi brand horizontal lathe without a guard to protect the operator from the rotating chuck.      c. In the machine shop where employees were exposed to struck-by and caught-by hazards when operating a Kingston brand manual horizontal lathe without a guard to protect the operator from the rotating chuck.      d. In the fabrication shop where employees were exposed to struck-by and caught-by hazards when operating a King brand vertical turret lathe without a guard to protect the operator from the rotating chuck.      e. In the fabrication shop where employees were exposed to struck-by and caught-by hazards when operating a Collet brand Horizontal Boring Mill without a guard to protect the operator from the rotating chuck.
