29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition. Call Floor: Employees were exposed to trip and fall hazards due to the carpet fraying and separating at multiple locations on the floor. On or about 09/16/2016, an employee tripped and fell onto a frayed section of carpet and sustained a dislocated knee and a sprain to the right foot requiring medical treatment from the incident.
