29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about October 19, 2016, employees were working around a shrink wrap machinery which did not have a guard around a pinch point area, exposing the employees to the hazard of caught in or amputation in the conveyor chain.

29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours: On or about 19 October, 2016, at the above addressed jobsite, an accident occurred at the work site which resulted in a hospitalization of an employee. The accident was not reported to the Fort Lauderdale Area Office of the Occupational Safety Health Administration within twenty-four (24) hours of the incident.
