Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly.    40 Francis St, Leominster, MA    On October 26, 2016 and at times prior, employees operated a powered industrial truck with two types of tine-mounted lifting attachments that reduced the truck's lifting capacity.   The employer did not have written approval of the manufacturer to use the two attachments.

The non-factory installed attachments were not identified on the data plate of the powered industrial truck.
