29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. On September 23, 2016 employees were exposed to a pinch point hazard on the AccurPress press brake located in the fabrication department. An employees thumb was amputated while he was holding the material being formed when his thumb was in between the material and the die on the machine.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. a) On September 23, 2016 part of an employees' thumb was amputated as a result of a work-related incident and not reported to OSHA within twenty-four hours.
