29 CFR 1926.453(b)(2)(iv): Employees did not stand firmly on the floor of the basket, and were seen to sit or climb on the edge of the basket for a work position:  (a)  Y and T Layout at 866 Kalamath, Denver, CO 80204: On and before October 26, 2016, the exposing employer failed to ensure that employees using a man lift stood firmly on the floor of the basket, and did not sit or climb on the edge of the basket or use planks, ladders, or other devices for a work position.  An employee was observed climbing out of the basket of the man lift into a second floor window to perform some work.  Condition exposed the employee to a fall hazard of approximately 20 feet.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   (a)  Y and T Layout at 866 Kalamath, Denver, CO 80204:  On and before October 26, 2016, the exposing employer did not ensure that employees performing framing on the second floor were protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  Condition exposed the employees laying out and attaching wooden decking to fall hazards of approximately 20 feet.
