OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the formwork was not erected and braced so that it was capable of supporting without failure all vertical and lateral loads that may reasonably be anticipated to be applied to the formwork:    On or about 10/26/2016, at the above addressed work site, an employee was exposed to a struck-by and fall hazard while placing forms around a reinforced steel column, in which the forms were not secured and the bracing was not properly anchored.    Among other feasible and acceptable methods to abate this hazard, the employer should:  a. Conduct regular inspections to ensure that each form is connected and secured prior to their release from the crane.  b. Inspect each brace to ensure that they are installed and secured in accordance with the manufacturers and/or engineers design specifications.  c. Establish a system in which employees clearly communicate to each other when the forms are secured, the bracing is properly anchored.
