29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.  a. On or about October 31, 2016, employees engaged in construction activities at the above referenced worksite were working from ladders, scaffolding, and aerial lifts. Ground personnel were not provided personal protective equipment to protect against falling / flying debris. Employees were thereby exposed to struck-by hazards.   Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1926.451(h)(2): Employees working below a scaffold were not protected from falling tools, materials or equipment in accordance with the provisions of paragraphs (h)(2)(i)-(v) of this section:  a. On or about October 31, 2016, employees where engaged in bricklaying activities from an elevated platform / scaffold. The area below this work zone was not barricaded or otherwise demarcated to control access into the danger zone. Employees were thereby exposed to struck-by hazards from falling tools, material, and/or debris.  b. On or about October 31, 2016, employees where engaged in bricklaying activities from an elevated platform / scaffold. There were no toe boards installed on the scaffolding edges. Employees were thereby exposed to struck-by hazards from falling tools, material, and/or debris.  Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access:   a. On or about October 31, 2016, employees of Burroak Masonry Inc. were required to work on an elevated platform in the performance of their duties. These employees were observed working from a scaffold platform greater than 2 feet to the next lower level without safe access as required by this standard. An employee of Burroak Masonry Inc. was observed utilizing cross bracing as a means of access to a scaffold platform. The employees were thereby exposed to fall from heights.   Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift.   a. On or about October 31, 2016, employees of Sauk Valley Builders were required to operate an aerial lift in the performance of their duties. These employees were operating the aerial lift without the required equipment to protect from falls, as outlined in this OSHA standard. Employees were thereby exposed employees to falls of approximately 12 feet.  Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   a. On or about October 31, 2016, employees of Burroak Masonry Inc. erected and were required to work from scaffolds placed an 12 foot high overhang with an unprotected edge. Access and egress from the scaffolding placed employees on an elevated walking working surface that required fall protection as outlined in this standard. The employees were thereby exposed to fall height of up to 12 feet to the ground level.   Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
