At the jobsite, the supervisor did not require employees to wear hard hats while they were building a concrete block wall and working on and around scaffolding, exposing employees to struck-by hazards, on or about 10/27/16.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:  a.  At the jobsite, the two tiered walk through type scaffolding employees were working from was not equipped with a ladder or safe means of access, resulting in employees climbing the end frames of the scaffolding, exposing the employees to a fall hazard, on or about 10/27/16.

At the jobsite, fall protection was not provided to employees building a concrete block wall while working from a two tiered scaffold, exposing employees to a 12 foot 11 inch fall hazard, on or about 10/27/16.
