29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed: (a) (CHEP) Millwood Atlanta BLUE facility, Pallet wash station: On or about October 19, 2016, and at times prior, the employer exposed employees to amputation hazards, in that guards were not provided or installed in locations where employees worked in close proximity to sprocket wheels and chains on the pallet wash line roller conveyor.
