29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch. On or about 26 October 2016, and at times prior thereto, employees used a pedestal grinder to sharpen drill bits in which the tongue guard was not adjusted to within 1/4 inch of the wheels surface, exposing the employees to the hazard of being struck by flying debris and projectiles.

On or about 26 October 2016, and at times prior thereto, two welders used hand-held buffers that were connected to an extension cord each which had visible strain relief damage, exposing the welders to an electrical hazard of approximately 120 volts.
