29 CFR 1903.19(c)(1): The employer did not certify to OSHA, within 10 calendar days after the abatement date, that the cited violation had been abated:  On December 1, 2016, Miller Building Supplies and Construction, was issued citations and the employer did not verify, through written certification that each citation was abated.  Abatement certification was not received for the following violations:              a.  Citation 1 Item 1 and,         b.  Citation 1 Item 2.

29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift:    On or about October 27, 2016, Miller Building Supplies and Construction, LLC had two employees exposed to an approximate fall of 8.5 ft. while working from a supported scaffold, which was lifted by a rough terrain forklift, and the supported scaffold was not secured to the rough terrain forklift by a chain or some other secure method, on a commercial construction site in Champaign, Illinois.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:      On or about October 27, 2016, Miller Building Supplies and Construction, LLC had two employees exposed to an approximate 8.5 ft. fall hazards while accessing and exiting the rough terrain forklift via a ladder that did not extend at least 3 ft. above the landing surface of the supported scaffold, on a commercial construction site in Champaign, Illinois.
