29 CFR 1926.602(d): The employer did not certify that each operator had been trained and evaluated as required by paragraph 29 CFR 1910.178(l): 29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace AND AS REFERENCED BY THE NOTE CONTAINED IN 1926.602(d): Building #2: On or about October 21, 2016, an employee was exposed to a stuck-by falling loads hazard and crushed by forklift tipping, while operating a JLG Rough Terrain Forklift, Model 10054, without being trained in the safe operation of the forklift.
