29 CFR 1910.333(a): Safety-related work practices shall be employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work is performed near or on equipment or circuits which are or may be energized. The specific safety-related work practices shall be consistent with the nature and extent of the associated electrical hazards. On or about August 31, 2016 through October 26, 2016, safety-related work practices were not employed to prevent electric shock or other injuries from either direct or indirect electrical contacts while employees were using the manual tester. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.335(a)(1)(i): Employees were not wearing protective equipment that is appropriate for the specific parts of the body to be protected and for the work to be performed when working in an area where there was a potential electrical hazard. On or about August 31, 2016 through October 26, 2016,employees were using the manual tester to perform product quality test on transformers and were not wearing proper protective equipment which resulted in electrical shock and burns. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
