29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.   (a)   The employer failed to provide documentation to the Agency demonstrating that abatement is completed for the serious violations that were issued by the Agency for lack of fall protection when working from scaffolds and aerial lifts, and fall protection hazard recognition training.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:    (a) The employer did not ensure that all employees were using fall protection when working from a ladder jack scaffold that was higher than 10', exposing employees to a fall hazard.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated:    (a) The employer did not ensure that all employees were using fall protection when working from the basket of an aerial lift.

(a) The employer did not ensure that all employees received training in topics such as, but not limited to, fall protection on a ladder jack scaffold and aerial lift system.
