In the maintenance shop area, employee engaged in grinding activities was exposed to struck-by hazard, in that the 6" Dayton bench grinder work rests was not adjusted to a 1/8th of an inch gap.

29 CFR 1910.178(a)(6): Nameplates or markings for powered industrial trucks were not in place or were not maintained in a legible condition: Employees operating a CLARK forklift MODEL: C3500-LP in the warehouse and production areas were exposed to struck-by hazards, in that one of the control levers which moves the forks was damaged and markings on the control lever were not in legible conditions to indicate which direction the operator should move the lever to avoid moving it in the opposite direction and striking someone or something.
