29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposes an employee to injury and the defective or damaged item was not removed from service: Machining Area, L08 Takisawa TS-20: On, or about, October 25, 2016, an extension cord used to energize a laptop was damaged in that the ground pin was missing from the male end and the outer insulation was pulled from the fitting on the female plug.
