29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  At the jobsite located at 602 W Stolley Park Dr, Grand Island, NE 68801 the employer is failing to protect employees from fall hazards of 23.5 feet from the scaffold platform to the ground below in that; the scaffold platform was not fully planked where employees were walking between the end section of the BilJax supported frame scaffold and the Hydromobile M2 scaffolding that was set up on the northwest side of the building.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.  At the jobsite located at 602 W Stolley Park Dr, Grand Island, NE 68801 the employer is failing to protect employees from fall hazards of 23.5 feet from the scaffold platform to the ground below in that; guardrails were not installed along the open side of the scaffold platform section between the end of the BilJax supported frame scaffold and the Hydromobile M2 scaffolding on the northwest side of the building, nor was any other means of fall protection provided to the employees working from and walking on this section of scaffolding.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  At the jobsite located at 602 W Stolley Park Dr, Grand Island, NE 68801 the employer is failing to protect employees from fall hazards of 16.5 feet from the roof deck to the ground below in that; employees performing masonry work while standing on the roof were not provided fall protection.
