On or about October 28, 2016, employees in the Middle Area/Barn exchanged 55-gallon drums of corrosive sodium hypochlorite solution (12.5-Percent; pH>10) that is used to treat the well water and the employer did not provide suitable facilities for quick drenching or flushing of the eyes for immediate emergency use.

On or about October 28, 2016, employees were exposed to electrical hazards and the employer did not install the following according to approved means:  a) grey liquid tight flexible conduit that powered the East exit sign of the Middle Area/Barn secured to a water pipe with a piece of wire.  b) drip proof box with a non-GFCI receptacle on the East wall of the DI Room that was installed below 3-inch Banjo valves that leak.   c) plastic zip ties to secure grey liquid tight flexible conduit (between the control panel and softener tanks along the East wall of the DI Room) to a piece of metal channel.  d) white PVC 90-degree elbows that are not approved for use as an electrical raceway serving the following:      - Wall mounted fan at the Northeast side of the Production Area      - Wall mounted fan at the Southeast side of the Production Area      - Wall mounted fan at the East side of the Middle Area/Barn      - Wall mounted fan at the West side of the Middle Area/Barn      - Switch at the West side of the Middle Area/Barn

On or about October 28, 2016, employees in the DI Room were exposed to electrical shock and burn hazards from:  a) a signal control panel in a water-tight type box suspended from Tank S2 and the box was cracked;  b) a Square D electrical disconnect switch behind Tank S3 and the switch was inaccessible because it was stuck inside the enclosure;  c) a Square D electrical disconnect switch behind Tank S3 and the enclosure was corroded by the purified water.
