29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed. Warehouse; on or about 26 October 2016 and times prior thereto; Two instances were noted where knockout plugs were missing on energized electrical outlet boxes. a. Office area, an energized electrical outlet was missing a knockout plug on the upper left side. b. Back wall adjacent to Powermatic drill press; an energized four place electrical outlet was missing a knockout plug on the top left hand side. This condition exposed employees to electrical shock and electrical burn hazard. The National Oceanic and Atmospheric Administration was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.305(b)(1)(ii), which was contained in OSHA inspection number 923117, citation number 1, item number 5 and was affirmed as a final order on 27 August 2013, with respect to a workplace located at 1211 Gibson Cove Road in Kodiak, Alaska.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). Warehouse and Room L209; on or about 26 October 2016 and times prior thereto; six instances were noted where electrical cords were damaged: a. Room L209; a PHI Hydraulic press, Model PW220H, serial number 96-8-007 had damaged to the outer jack insulation. b. Warehouse Bay 2; a Porter Cable Porter 6" Random Orbital Sander, Model 7336, Serial number 119250A8920 had damaged to the outer jack insulation. c. Warehouse Bay 2; a Makita 5" Disc Sander, Model GV5000, serial number 327317 had damage to the outer insulation jacket. d. Warehouse Bay 2; a Dewalt 1/2" VSR Drill, Model DW110, Serial number 6690 had damage to the outer insulation jacket. e. Warehouse Bay 2; a Milwaukee 16 gauge Nibbler, Model 6890, serial number not legible, had damage to the outer insulation jacket. f. Warehouse Bay 2; A Dewalt 1/2" Impact Wrench, Model DW290, serial number 356166, had damage to the outer insulation jacket. This condition exposed employees to an electrical shock hazard. The National Oceanic and Atmospheric Administration was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.334(a)(2)(i), which was contained in OSHA inspection number 1165311, citation number 2, item number 4 and was affirmed as a final order on 26 August 2016, with respect to a workplace located at the Anchorage Forecast Office Maintenance shop.
