29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use First Floor Hallway, Outside HAZMAT (Rm L114) and Ichthyic Lab (Rm L107): On or about 26 October 2016 and times prior thereto, the emergency eye and body wash was blocked with cleaning/janitorial equipment and supplies. This condition exposed employees to eye trauma hazards.

29 CFR 1910.1200(f)(5): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged, or marked with the identity and appropriate hazard warnings regarding the chemicals contained therein. TSMRI, Wet Lab, Room L119: On or about 26 October 2016 and times prior thereto, unattended 20 oz. plastic bottle; containing an unknown chemical substance was not marked identifying the product and the hazard warnings associated with it. This condition exposed employees to chemical burns, poisoning, and skin dermatitis.
