29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i).  Building 1108, adjacent to roll-up door; on or about 26 October 2016 and times prior thereto; a 120 volt energized electrical outlet was not guarded, exposing live electrical wires. This condition exposed employees to an electrical shock and electrical burn hazards.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed.  Warehouse and shops; on or about 26 October 2016 and times prior thereto; Four instances were noted where knockout plugs were missing on energized electrical outlet boxes.        a. Machine shop; adjacent to 1104 doorway, a knockout plug missing on right hand side of 240 volt electrical box adjacent to panel board.         b. Boat house; a knockout plug was missing on the right hand side and on the top of the electrical panel board adjacent to the paddle fan light switch.        c. Machine shop back wall of 1103, a 240 volt safety switch box was missing a knockout plug on the top of the box.  This condition exposed employees to electrical shock and electrical burn hazard.  The National Oceanic and Atmospheric Administration was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.305(b)(1)(ii), which was contained in OSHA inspection number 923117, citation number 1, item number 5 and was affirmed as a final order on 27 August 2013, with respect to a workplace located at 1211 Gibson Cove Road in Kodiak, Alaska.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket).  Machine shop area; on or about 26 October 2016 and times prior thereto; a Milwaukee Heavy Duty 14" Abrasive Cut-Off Machine, Model 6175, Serial number 96493299 had damage to the outer insulation jacket.  The National Oceanic and Atmospheric Administration was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.334(a)(2)(i), which was contained in OSHA inspection number 1165311, citation number 2, item number 4 and was affirmed as a final order on 26 August 2016, with respect to a workplace located at the Anchorage Forecast Office Maintenance shop.

29 CFR 1910.23(a)(9): Every floor hole into which persons cannot accidentally walk (on account of fixed machinery, equipment, or walls) shall be protected by a cover that leaves no openings more than 1 inch wide. The cover shall be securely held in place to prevent tools or materials from falling through.  Wood shop area; on or about 26 October 2016 and times prior thereto; a floor hole measuring approximately 4 1/2 inches, with a depth of approximately 52 inches was not guarded.  This condition exposed employees to a tripping hazard.
