29 CFR 1926.21(b)(5): Employee(s) required to handle or use flammable liquids, gases, or toxic materials were not instructed in the safe handling and use of these materials and made aware of the specific requirements contained in 29 CFR 1926, subparts D.    a. On or about October 28, 2016 employees were applying a combustible silicone roof coating blended with a highly flammable reducer and were not trained by the employer on how to safely handle, and use these products, exposing employees to burn hazards.    In the alternative 29 CFR 1910.1200(g)(8):  The employer did not maintain in the workplace copies of the required safety data sheets for each hazardous chemical, and did not ensure that they were readily accessible during each work  shift to employees when they were in their work area(s).

29 CFR 1926.152(e)(2): The transfer of flammable liquids from one container to another was done when the containers were not electrically interconnected (bonded).    a. On or about October 28, 2016 employees transferring flammable liquids, like reducer from 1 gallon cans, and silicone paint into a 5 gallon plastic bucket were exposed to static charges and flash fires because the 5 gallon plastic bucket they were using was not grounded or effectively bounded to the gasoline operated paint sprayer or grounded to the earth, to prevent discharge of static electricity.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.      a. On or about October 28, 2016 employees had placed a portable A frame fiberglass ladder to gain access to a roof and did not have the top of the ladder extend at least 3 feet above the point of the roofline.
