29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. a) On or about 10/27/2016 at the front of building # 6918, the second level of the scaffold, guardrails system was missing on the scaffold. Employees were leveling/scrapping the foam insulation on the building walls exposed to a 13 ft. fall hazard.

29 CFR 1926.451(h)(2)(ii): A toeboard was not erected along the edge of platforms more than 10 feet (3.1 m) above lower levels for a distance sufficient to protect employees below: a) On or about 10/27/2016 at the front of building # 6918, the second level of the scaffold, toeboards were missing on the scaffold. Employees on the lower level were exposed to a struck-by hazard.
