29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about October 28, 2016, and in times prior thereto, workers standing on the ground beneath other workers and installing siding on apartment building did not use head protection exposing them to hazard of being struck by falling tools and building materials.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about October 28, 2016, and in times prior thereto, worker putting on siding while standing on a roof did not have fall protection. This condition exposed workers to the hazard of falling off of the roof and striking the ground or walkways 30 feet below.
