29 CFR 1910.333(b)(2)(ii)(B): The circuits and equipment to be worked on were not disconnected from all electric energy sources: On or about October 20, 2016, the employer failed to ensure that an electrical ballast in an overhead lighting fixture was disconnected from its energy sources prior to work being performed. The circuit breaker of the 277 volt system was not operated in a manner to isolate the electrical energy source from the circuit to the lighting ballast.
