29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided. Worksite: The employer did not ensure that the employee who was working inside of the energized electrical disconnect box was provided with the appropriate personal protective equipment (PPE) to protect the employee from electrical hazards, such as (but not limited to) electrical shocks, arc flash/arc blast and/or an electrocution. On or about 09/29/2016, the above employee came in contact with energized electrical parts while performing the task and received electrical shocks.

29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means. Worksite: The employer permitted employees to work on and in very close proximity to an energized electrical disconnect box. On or about 09/29/2016, one of employees came in contact with energized electrical parts and received electrical shocks while working inside of the energized electrical disconnect box.
