29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact.    Or in the alterative:    29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed:    (a) Location - Electrical Room, Charles Square Hotel:  On or about 10/17/16, employees were not provided with and did not use personal protective equipment (PPE) while working with energized electrical equipment.  Employee was working in a energized 5,000 amp panel and did not use any PPE.

29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:    or in the alterative    29 CFR 1910.333(a)(1): The employer failed to ensure that live parts were de-energized before employees began to work on or near live parts.    (a) Location - Electrical room, Charles Square Hotel:  On or about 10/27/16, the employer failed to ensure that safety related work practices (consistent with the nature and extent of the associated electrical hazard) were used by employees while they were working on energized electrical equipment.

29 CFR 1926.417(a): Controls that were to be deactivated during the course of work on energized or de-energized equipment or circuits were not tagged:  Or in the alterative     29 CFR 1910.333(b)(2)(ii)(A): Safe procedures for de-energizing circuits and equipment were not determined before circuits and equipment were de-energized.    (a) Location - Electrical room, Charles Square Hotel:  On or about 10/27/16, the employer failed to ensure employees were de-energizing electrical circuits and did not lock or tag out electrical circuits. The employer failed to ensure employees were protected from energized electrical parts while installing a new switch for a new transformer.
