29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about October 27, 2016, employees work and clean near the hazardous locations.  Employees were exposed to struck-by hazards when the following were not guarded or were not adequately guarded: a. the expanded metal guard over the twist mechanism on baler #2 was broken b. the guard over the sword/blade that transported the wire to the operator side of baler #1 permitted access to the sword/blade as it exited the guard and entered the compressed materials c. the guard over the sword/blade that transported the wire to the operator side of baler #2 permitted access to the sword/blade as it exited the guard and entered the compressed materials d. the guard over the twist mechanism permitted access to the sword/blade as it exited the compressed material and before it entered the guard.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
