29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.      a On or about October 31, 2016 employees had set a ladder at the rear of the structure and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):     a. Employees installing a new metal roof at elevations greater than 10 feet above the ground are not being protected from fall hazards with the use of fall protections systems. The employer fails to assure effective protection against falls as required by the standard, as demonstrated on October 31, 2016, when one employee installing new metal valley materials and installing new waterproof underlayment on a 2:12 pitch low slope roof of a residential structure located at 1301 SE 3rd Ave, Gainesville, FL are exposed to the hazard greater than an 10 foot fall to the ground in that they do not have roof anchor points, lanyards, personal fall arrest gear, guardrails or safety nets.    Atlantic Roofing & Exteriors LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1025152, citation number 1, item number 1 and was affirmed as a final order on March 17, 2015, with respect to a workplace located at 4121 Huntington Forest Dr. Jacksonville, FL 32257.
