29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: (a) Plant 6, South Cup - While troubleshooting mechanical problems with machinery, including, but not limited to the conveyor for the bagger, cup mechanics removed guarding that enclosed moving machine parts, such as chain and sprocket drives, without applying lockout/tagout, exposing these employees to getting caught in or caught by moving machine parts, on or about October 25, 2016. Corrected during the inspection.
