29 CFR 1926.451(b)(4): Each end of a platform, which was not cleated or otherwise restrained by hooks or equivalent means, did not extend over the centerline of its support at least 6 inches: a) For employees working at 384 Aruba Circle in Bradenton as observed on or about 27 October, 2016. Employees were working on scaffolding where the planking was not extended a minimum of six inches over the support. The planking was not pinned nor restrained. Employees exposed to a fall hazard of approximately 5 feet.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: a) For employees working at 384 Aruba Circle in Bradenton as observed on or about 27 October, 2016. Employees were using extension cords to supply power to tools that were missing the path to ground. The ground pins were broken off.
