29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupers for personal protection.   a) For employees working at 209 82nd Street in Holmes Beach, as observed on or about October 27, 2016. Employees were using power tools with extension cords that did not have ground fault circuit interrupter protection.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:  a) For employees working at 209 82nd Street in Holmes Beach, as observed on or about October 27, 2016. Employees were using power tools connected to an outlet that was not grounded.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a) For employees working at 209 82nd Street in Holmes Beach, as observed on or about October 27, 2016. Employees were installing roofing material on a residential roof without the use of fall protection system in place. Employees were exposed to a fall hazard of approximately 9 feet.

29 CFR 1926.51(a)(4): The common drinking cup was not prohibited:  a) For employees working at 209 82nd Street in Holmes Beach, as observed on or about October 27, 2016. Employees were drinking out of a water container without the use of individual cups. The employees were holding the container up and drinking right out of the common tap.

Employees were working on a residential roof and the employer did not have a training certification record to verify compliance of training received.
