Willow Creek Companies at 5480 Grass Mesa Road, Rifle, CO 81650: On October 26, 2016, the workplace contained a struck-by hazard to which an employee of the employer was exposed while staging and positioning 60 foot joints of 12 inch steel pipe, in that pipe joints stacked on wood crotches became unsecured, when after being initially secured, the securing wood chock of the securing crotch was improperly dislodged by the employee, causing the joints to roll, fatally striking the employee. This activity exposed the employee to a struck-by hazard.
