29 CFR 1910.27(f): "Maintenance." All ladders were not maintained in a safe condition. All ladders shall be inspected regularly, with the intervals between inspections being determined by use and exposure: a) Facility, 10228 Lincoln Highway, Everett, PA: On or about October 31, 2016, the daily pre-trip inspection for truck #20 had not been performed prior to each trip, and the bottom rung on the ladder attached to the back of it has been rusted through and broken for the at least the past three years.
